Instructions for completing the application

Fill out the forms accurately and completely.

            The application packet consists of the following forms:

-    Information Form (1 page)

-    MIS Application for Employment (3 pages)          

-    Background Check Consent Form (1 page)

-    MIS Workplace Substance Abuse Policy (1 page)

-    Informed Consent and Release of Liability (1 page)

-    Employee Medical History Questionnaire (2 pages)

-    I9 Employment Eligibility Verification – Fill out Section 1 on page 4 only (5 pages)

-    Form W4 (2011) Federal Withholdings (2 pages)

-    Form L4 State Withholdings (1 page)

Once completed, submit a copy via fax, mail or email.  

You must also send a copy of your drivers license or other form of identification as well as documents that establish employment authorization.  See page 5 of I9 Employment Eligibility Verification Form for a list of acceptable documents. We will not be able to process your application without these documents. Send documents via fax, email or mail.
