Instructions for completing the application
You may want to print these instructions for your reference prior to beginning the application.
When you click on the APPLICATION link a PDF file will open in a new window. When you begin to fill in the forms you will see the following information box appear. This information box is only to inform you that a data file will be emailed once the submit button is clicked and that you cannot save a completed copy of the forms. Click Close.
Fill out the forms accurately and completely.
The application packet consists of the following forms:
- Information Form (1 page)
- MIS Application for Employment (3 pages)
- Background Check Consent Form (1 page)
- MIS Workplace Substance Abuse Policy (1 page)
- Informed Consent and Release of Liability (1 page)
- Employee Medical History Questionnaire (2 pages)
- I9 Employment Eligibility Verification – Fill out Section 1 on page 4 only (5 pages)
- Form W4 (2010) Federal Withholdings (2 pages)
- Form L4 (2010) State Withholdings (1 page)
- Electronic Signature and Electronic Delivery of Disclosures and Notices (1 page)
Once completed, print a copy to retain for your records and click the SUBMIT button found on the last page. You may also choose to print and send a copy via fax or mail if you encounter problems using the submit button.
When you click the submit button you will see the following information box appear.
Choose the option that best fits the email program you will use to submit the data file.
o If using Desktop Email Application, select this option and click OK. A new email message will appear with the data file attached. Click Send.
o If using Internet Email, select this option and click OK. A box to save the data file will appear (as shown below), save the file to your desktop. Open your internet email account and begin a new email, attach the data file you saved on your desktop to the email and click send.
You must also send a copy of your drivers license or other form of identification as well as documents that establish employment authorization. See page 5 of I9 Employment Eligibility Verification Form for a list of acceptable documents. We will not be able to process your application without these documents. Send documents via fax, email or mail.
